About the building
Sir John Mason House opened in 2015 and is home to the De Lacy Wellbeing Community Hub, the Community Support Team and the Intermediate Care Centre.
- De Lacy Community Wellbeing Hub provides advice and support to vulnerable adults. There is a range of activities held at the hub each week.
- The Community Support Team provides short term help at home. They offer specialist home care services across North Lincolnshire and support vulnerable adults and older people to help them live independently in their own home and lead a fulfilling life. - The Intermediate Care Centre provides rehabilitation and reablement programmes to people who are not well enough to live in their homes after a period of illness or injury. Intermediate Care can also be provided in a person's own home. Please see our Getting Well Again page for more information. A charge for Intermediate Care Services will be made after six weeks.
